Summary
To support our GP Surgery with LTC (Long Term Conditions) and QOF management (Quality and Outcomes Framework)
Duties
- Maintain and update patient records accurately.
- Input clinical data for QOF reporting and assist with tracking targets.
- Schedule and manage patient appointments for LTC reviews.
- Handle patient queries and coordinate with clinical staff.
- Prepare and manage reports related to LTC and QOF and update at monthly team meetings
Skills & Experience
Essential:
- Administrative experience in QOF and LTC, preferably in healthcare.
- Strong organizational skills and attention to detail.
- Proficient in Microsoft Office and Emis healthcare software
- Excellent communication and confidentiality awareness.
Desirable:
- Knowledge of QOF and Long-Term Conditions.
- Experience in a GP surgery or NHS environment
